Why do my extracted frames have horizontal lines through them?

Because the source is interlaced. MPG content from the broadcast and DVD era stores two half-height fields per frame, captured a fiftieth of a second apart, and a still pulled straight out shows both woven together as combing on anything that moved. Enable deinterlacing before extraction and each still is built from a single reconstructed progressive frame. MPG is the same MPEG program stream as .mpeg — the three-letter extension is a DOS-era truncation, not a different format.
Which frame of the MPG do I actually get as a JPEG?

The one you point at, not just the first. A timestamp picks a single moment out of the MPG; an interval setting walks the whole video and returns one JPEG per step, delivered as a ZIP. The default grabs a frame a few seconds in rather than at 00:00, because the opening frame of a video is very often black.
What quality setting should I use for the JPEG output?

Quality 85 is the sensible default and the one used unless you change it — visually indistinguishable from the source at normal viewing distance, at roughly a fifth of the size of a lossless equivalent. Push to 95 for anything that will be edited or printed, and drop to 70 for thumbnails where size dominates. The bitstream is identical to a .jpg: DCT-based lossy compression, usually with 4:2:0 chroma subsampling, wrapped in a JFIF or Exif header.
